In recent years, computational fluid dynamics CFD has become a valuable tool for investigating hemodynamics in cerebral aneurysms. CFD provides flow-related quantities, which have been shown to have a potential impact on aneurysm growth and risk of rupture. However, the adoption of CFD tools in clinical settings is currently limited by the high computational cost and the engineering expertise required for employing these tools, e. Herein, we address these challenges by introducing a practical and robust methodology, focusing on computational performance and minimizing user interaction through automated parameter selection. We propose a fully automated pipeline that covers the steps from a patient-specific anatomical model to results, based on a fast, graphics processing unit- GPU- accelerated CFD solver and a parameter selection methodology. We use a reduced order model to compute the initial estimates of the spatial and temporal resolutions and an iterative approach that further adjusts the resolution during the simulation without user interaction.

Intracranial aneurysms are associated with disturbed velocity patterns, and chronic inflammation, but the relevance for these findings are currently unknown. Here, we show that disturbed shear stress induced by vortices is a sufficient condition to activate the endothelial NF-kB pathway, possibly through a mechanism of mechanosensor de-activation. We provide evidence for this statement through in-vitro live cell imaging of NF-kB in HUVECs exposed to different flow conditions, stochastic modelling of flow induced NF-kB activation and induction of disturbed flow in mouse carotid arteries.

In virtual stenting, a flow-diverting mesh tube stent is modeled inside the reconstructed vasculature and integrated in the simulation. We focus on steady-state simulation and the resulting complex multiparameter data.

Computational fluid dynamics CFD has been used for several years to identify mechanical risk factors associated with aneurysmal evolution and rupture as well as to understand flow characteristics before and after surgical treatments in order to help the clinical decision making process. This search enables us to examine the current status of CFD as a clinical tool and to determine if CFD can potentially become an important part of the routine clinical practice for the evaluation and treatment of aneurysms in near future. According to previous reports, it has been argued that CFD has become a quite robust non-invasive tool for the evaluation of surgical devices, especially in the early stages of device design and it has also been applied successfully to the study of rupture risk assessment. However, we find that due to the large number of pre-processing inputs further efforts of validation and reproducibility of CFD with larger clinical datasets are still essential to identify standardized mechanical risk factors. As a result, we identify the following needs to have a robust CFD tool for clinical use: i more reliability tests through validation studies, ii analyses of larger generalized clinical datasets to find converging universal risk parameters, iii fluid structure interaction FSI analyses to better understand the detailed vascular remodeling processes associated with aneurysm growth, evolution and rupture, and iv better coordinated and organized communications and collaborations between engineers and clinicians. An Automated Workflow for Hemodynamic Computations in Cerebral Aneurysms

About a decade ago, the first image-based computational hemodynamic studies of cerebral aneurysms were presented. Their potential for clinical applications was the result of a right combination of medical image processing, vascular reconstruction, and grid generation techniques used to reconstruct personalized domains for computational fluid and solid dynamics solvers and data analysis and visualization techniques. A considerable number of studies have captivated the attention of clinicians, neurosurgeons, and neuroradiologists, who realized the ability of those tools to help in understanding the role played by hemodynamics in the natural history and management of intracranial aneurysms. This paper intends to summarize the most relevant results in the field reported during the last years.
